Clinical Document Reviewer (01)Should be able to study patients Medical condition and design care plans Conduct care team meetings Internal monitoring of the data and suggesting tactics to improve care and achieve practice goals. Should be able to analyze patient data recorded in software and define the care plans for high - risk chronic disease patients. Define Risk level of patient population based on their Medical condition. Should be able to Communicate with Doctors Office on phone call or via email to coordinate the patient care.
