Job Description

    Job SummaryThe Patient Care Coordinator is responsible for coordinating and supporting patients throughout their healthcare journey. This role serves as a key point of contact between patients, healthcare providers, and administrative teams to ensure timely appointments, effective communication, accurate documentation, and a positive patient experience.Key Responsibilities
    1. Coordinate patient appointments, consultations, follow-ups, and procedures.
    2. Communicate with patients regarding appointments, treatment plans, and healthcare services.
    3. Assist patients with registration, referrals, insurance, and required documentation.
    4. Coordinate communication between patients, doctors, nurses, and other healthcare professionals.
    5. Follow up with patients regarding appointments, test results, and ongoing care needs.
    6. Maintain accurate and confidential patient records in the EMR/EHR system.
    7. Address patient concerns and resolve administrative or service-related issues promptly.
    8. Ensure a smooth patient experience by coordinating care and supporting efficient patient flow.
