Education & Experience: Graduate/Post-graduate in Nutrition, Nursing, Pharmacy or any other relevant clinical field. Experience with medium to large participant load at a time (300+ participants/coach). Demonstrated prior (minimum 1year+) experience working with behavior change or chronic disease management, including but not limited to diabetes, hypertension, weight loss & nutrition. Key Responsibilities: Reach out to participants to understand their health goals and gaps in care, identify and skillfully demonstrate how the Wellthy Care program can fulfill this need (minimum 50 + outbound calls per day). Influence users to appreciate the value of the program and persuade them to purchase a paid plan at the end of the limited duration free- trial period. Focus on serving the program participants efficiently and effectively to achieve the best possible outcomes within the free trial period as well as afterwards (for paid users). Provide real-time support and response to the participants Actively engage the participants through a variety of channels including telephone and in-app chat, to provide feedback and support, answer questions, share resources and hold them accountable for their goals. Build and maintain a positive coaching relationship with an assigned caseload of participants,motivating and empowering them to get the most out of the program to reach their health goals. Coaching activities include providing education on the condition, demonstrating the need for holisticmanagement and creating customized diet and lifestyle solutions to help participants maximize theprogram offerings and achieve relevant clinical outcomes. Assess and note participant progress and status while tracking health data and factors such asbehavior changes. Analyze user data and behavior, critically assess the coaching approaches, and relay insights andfeedback to the Product and Engagement Teams. Convey enthusiasm and positive energy during participant interactions.Specific Job Requirements: Comfortable working in a remote, virtual environment using Google Meet, Zoom and othercommunication platforms. Highly developed skill sets with regard to reflective listening, rapport building, establishing trust andmodeling empathy Ability to present information clearly and effectively when speaking with others Prior experience and/or a high comfort level using software/technology to deliver health coaching. Comfortable making outbound calls to participants and talking to new people.JOBDESCRIPTION Experience working with chronic condition management, such as diabetes, hypertension and weight management is a plus. Exceptionally detail oriented and organized.
