Roles and Responsibilities : Medical Care

  1. Monitor and implement nursing procedures and practices.
  2. Respond to possible daily complications developed by patients during dialysis procedure and compile monthly statistics for all patients
  3. Educate patients and staff on the procedures and protocols of dialysis therapy.
  4. Ensure that reprocessing of dialyser is done according to P&P.
  5. Maintain adequate stocks of all dialysis consumables and catheters; obtain necessary approval from Dialysis incharge for replenishing consumables
  6. Schedule dialysis appointment for patients based on availability, duration, special conditions (positive /negative status etc.)
  7. Maintain all the instruments in working condition; obtain necessary approval form
  8. If any machine breaks down, report to the Service Engineer, Nephrologists and Dialysis in charge. Ensure that the machine is serviced and it is safe to proceed with the treatment.
  9. Co-ordinate with respective vendors for servicing and repair of equipment.
  10. Prepare dialyser for patients in the subsequent shifts.
  11. Remind all the patients about the date of subsequent blood investigation in advance.
  12. Ensure the drugs and store keys are properly secured.
  13. Ensure previous days records are updated.
Patient Care
  1. Monitor attendance regularly; and make provisions for back-up staff in case of leave of absence so as to ensure continuity of medical and patient care.
  2. Work with the unit - in- charge in finding out the root cause of the complications that occurred to the patient during dialysis by providing sufficient details.
  3. Ensure that all the machines are disinfected and enter in the system.
  4. To ensure RO readings are in safe limits and water level is maintained for the subsequent shift.
  5. Before starting Hemodialysis, check prescription and order from the Nephrologist; ensure treatment and patient conditions are in line with the order.
  6. Conduct a check of relevant bills.
  7. To interact with all the patients and note down the complications pre- HD, during HD and post HD if, any; inform the Nephrologist during rounds.
  8. Check all the blood reports and update in the Electronic Medical Reports.
  9. Educate patients if, found out of range blood values.
  10. Highlight all abnormal values in rounds and as per Nephrologists advice, medication to be given.
  11. Perform patient assessment, develop and administer dialysis plans and medication.
  12. Perform Fluid Therapy/Blood product and other treatments according to physician orders.
Process and Quality
  1. Update patient history and dialysis data in Electronic Medical Reports and/ or specific files for current and future reference.
  2. Provide inputs in formulating patient care policy and procedures, for patient care service and the organization.
  3. Ensure adherence to NABH standards and other relevant quality standards in all processes.
Staff and Facility Management1. Assist Team mates in preparing patient schedules. 2. Training new team mates for preparing, monitoring and maintaining dialysis machines and system. 3. Monitor all the staff whether they are following the unit protocol if, not, report to the Unit in- charge. 4. Ensure that reprocessing / dilutions etc. are done according to unit procedures and protocol. 5. Ensure the facilities are provided for emergency resuscitation. 6. To maintain the minimum stock in the emergency trolley. 7. Check the emergency trolley on daily basis and ensure all the items are available. 8. Ensure overall hygiene and waste management process, as per protocol within the dialysis unit. 9. Train and mentor junior staff; assist in preparing content for training; orientate them in the protocols and procedures of the dialysis unit. 10. Work along with clinical instructors in planning, implementation and evaluation of staff clinical experience. 11. Responsible for completion of regular performance evaluations for all direct reports; conduct evaluation for new staff prior to completion of probationary period. 12. Plan and maintain adequate staffing in the unit so as to ensure continuity of medical care at all times.
