Job Description

• Transcribing patient informationMedical coders review medical documents and records, such as physicians notes, lab reports, procedures, diagnoses, or equipment, and translate the information into medical codes.These codes may include diagnosis codes, treatment/services codes, and medical hardware codes.
• Ensuring accuracyMedical coders make sure that codes are assigned correctly, and that all documents are grammatically correct.They also examine medical documents and records for missing information, and identify documentation deficiencies.
• Complying with guidelinesMedical coders must adhere to medical coding guidelines and policies, and ensure that documentation supports diagnoses, procedures, and treatment results.
• Communicating with othersMedical coders communicate with medical providers and insurance companies to confirm details and code to the highest degree of specificity .They also serve as a resource and subject matter expert to other coding staff.
Other responsibilities of medical coders may include:
• Conducting chart audits
• Collecting, researching, and filing patient medical information
• Analyzing data needs for reimbursement
