Job Description

    Job descriptionWe are seeking a dedicated Licensed Practical Nurse to join our team at Atria Greenridge Place. The ideal candidate will have experience in various medical settings working with seniors and possess a strong passion for patient care.Essential Duties and responsibilities include, but are not limited to:
    • Ensuring the well-being of residents by delivering high-quality nursing care consistent with the assisted living philosophy and state regulations.
    • Supporting the Director of Resident Services in overseeing the activities of CNAs and assisting with resident self-administered medication programs.
    • Communicating efficiently with residents, families, and colleagues to address concerns and provide assistance as needed.
    • Maintaining accurate records and documentation related to resident care and medication administration.
    • Collaborating with the healthcare team to develop and implement individualized care plans for residents in ongoing training and professional development to enhance skills and knowledge in geriatric care.
    • Participation in assessments under the supervision of the Resident Care Director.
    • Assist in the training of associates and auditing of care.
    • Collaborate & maintain communications with physicians, labs and home care agencies to ensure strong coordination of care.
    • Complete monthly in-service training as required.
    • Participate in meetings to discuss needs and requirements of each Resident and take appropriate action as needed and directed to promote quality Resident Care.
