Job Description

: Behavioral Health Case Manager Springfield, MOOptum Home & Community Care, part of the UnitedHealth Group family of businesses, is creating something new in health care. This preventive care can help patients stay well at home.This is a field-based position covering the greater Springfield, MO area. You will be in the field approximately 75 PERCENT of the time going into members homes. You will be working from home when not in the field. The cities in your territory include but are not limited to: Salem, Rolla, West Plains, Springfield, Van Buren, Mountain Grove, and Willow Springs. No on-call, no weekends, and no holidays required.As a Case Manager in the United Healthcare at Home program, you will provide social work services to members in a home setting. The Case Manager provides intervention, assessment, and consultation of social, functional, and behavioral health issues affecting chronically ill and high-risk patients in a variety of environments, primarily the home setting.The Case Manager works in collaboration with a multidisciplinary team to identify and access services to close care gaps and improve the quality of life of OAH members. They help to manage health problems and coordinate health care services for the patients in accordance with State and Federal rules and regulations. This includes (but is not limited to) assessment, development, implementation of a plan of care, and ongoing evaluation of patient status and response to the plan of care. Ongoing care and management of members is conducted in collaboration with other care team members.This position is full-time (40 hours/week) Monday Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00 am 5:00 pm. It may be necessary, given the business need, to work occasional overtime. This position is a field-based position with a home-based office. You will work from home when not in the field.If you reside near Springfield, MO, you will have the flexibility to work remotely* as you take on some tough challenges.Primary Responsibilities:

  • Completion of Biopsychosocial Assessments
  • Case management/care management/care coordination/health care navigation
  • Discharge and transition planning
  • Client and family engagement in all aspects of social work intervention
  • Advanced healthcare planning
  • Palliative care, including pain management
  • Hospice and End-of-life care
  • Crisis Intervention
  • Identification of elder/vulnerable adult abuse, trauma, neglect, and exploitation
  • Facilitation of benefits and resource acquisition to assist clients and families, including an understanding of related policies, eligibility requirements, and financial and legal issues

  • Licensed Masters degree in Psychology, Social Work, Counseling, or Marriage or Family Counseling, or Licensed Ph.D., or an RN with 2 years of experience in behavioral health
  • Active, independent, and unrestricted licenses in the state of Missouri
  • 2 years of experience in a related mental health setting
  • 2 years of experience working with Microsoft Word (creating, editing, saving, formatting), Microsoft Excel (creating and editing spreadsheets), and Microsoft Outlook (composing and responding to email)
  • Dedicated work area established that is separated from other living areas and provides information privacy
  • Demonstrated ability to keep all company sensitive documents secure (if applicable)
  • Willing or ability to travel up to 75 PERCENT in the territory listed
  • Willing or ability to work Monday-Friday any of our 8-hour shift schedules during our normal business hours of 8:00 am 5:00 pm. It may be necessary, given the business need, to work occasional overtime
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
  • Reside within 90 minutes from Springfield, MO
  • Valid drivers license
  • Reliable transportation
Preferred Qualifications:
  • 2 years experience in long-term care, home health, hospice, public health, or assisted living
  • 1 years of clinical or case management experience
  • Experience with electronic charting
  • Experience with arranging community resources
  • Field-based work experience going into homes
  • Background in managing populations with complex medical or behavioral needs
